Minimum to Midpoint Pay Rate:$38.48 - $50.01 / hour

Summary

**This position is remote, but will require some travel between locations**

This position is responsible for providing day to day operational leadership for the LPG Community Care Outreach team. These responsibilities include staff orientation and workflow oversight, caseload management and team development. The manager serves as a role model inspiring individual and organizational excellence and creates a shared vision to attain the organizations mission and strategic goals. This role collaborates with Lee Health leaders, providers and community partners as appropriate. This leader is responsible for impacting and providing the right care at the right time and place.

Requirements

Education: Minimum of bachelors degree in social sciences (BSW) or, graduate of an accredited school of nursing with baccalaureate degree in nursing (BSN). Masters degree in nursing (MSN) or social sciences (MSW) preferred

Experience: 3 Years Care Management or Community Home Care Management required.

Certification: N/A

License: Florida RN (If nursing degree)

Other: Ability to creatively problem solve and access resources. Excellent interpersonal, communication, negotiation, conflict resolution and planning skills. Leadership skills, including demonstrated ability to influence leaders.
